Your electronic health record (EHR) transition is underway. Executive teams are focused on timelines, implementation milestones, training plans, and go-live readiness. The future-state platform commands attention, and rightly so.
But for many health systems, the most material risk is not the system they are moving toward. It is the current-state environment that continues to support patient care, clinical workflows, revenue cycle operations, reporting needs, interfaces, and regulatory requirements during the transition.
Innovation is often framed as what comes next. In practice, innovation only creates value when an organization can maintain control over what is happening now. Future-state technology improvements depend on present-state stability, governance, and visibility.

Health systems often treat EHR transitions as transformation initiatives. Implementation milestones, adoption rates, and go-live dates measure success. Those metrics matter, but they can also create blind spots.
A common assumption is that current-state systems need only be maintained until the transition is complete. As long as the lights stay on, the organization can focus on the future. The reality is more complicated.
Current-state systems remain mission-critical throughout the transition. They continue to support clinical operations, financial processes, regulatory requirements, and access to historical data. When support models weaken, the consequences are rarely immediate but often significant.
The challenge is not that organizations overestimate the importance of the future-state platform. It is that they underestimate their continued dependence on the systems that still run the organization today.
What Happens Behind the Scenes
The transition period creates pressure from multiple directions. Internal teams that have spent years building expertise in existing platforms are often reassigned to implementation work. Institutional knowledge begins to erode from the day-to-day support model. At the same time, application ecosystems remain as complex as before the transition began.
In many organizations, ticket volumes remain steady or increase. Clinical, ancillary, and revenue cycle applications continue to require specialized support. Dual-system environments add operational complexity, and internal teams are asked to do more with fewer dedicated resources.
In our work with health systems, one of the most common risks we see during major EHR transitions is the redirection of internal subject matter experts to the implementation effort. At the same time, current-state ticket volumes, clinical workflows, revenue cycle processes, reporting needs, and interface dependencies remain steady. The result is not a single major failure point. Instead, it brings slower issue resolution, growing backlogs, reduced visibility, more workarounds, and greater operational friction at exactly the time the organization needs more control, not less.
These issues rarely make headlines during a transition, but collectively they can have meaningful operational and financial consequences. Problems become harder to identify, prioritize, and resolve. At a time when healthcare organizations continue to face margin pressure, even small disruptions can have outsized consequences for cash flow, resource allocation, and organizational stability.
What High-Performing Health Systems Do Differently
The organizations that navigate EHR transitions most successfully treat current-state support as a strategic function rather than a temporary necessity.
Successful organizations:
- Align support resources around specialized domain expertise, including clinical, revenue cycle, ancillary, reporting, and interface support.
- Preserve and document institutional knowledge before key subject matter experts are reassigned to implementation work.
- Maintain structured support models with clear ownership, escalation paths, and governance cadence.
- Establish incident management and SLA expectations that ensure accountability across current-state and future-state workstreams.
- Continue targeted optimization instead of limiting activity to basic maintenance.
- Monitor performance metrics across both current-state and future-state environments.
Rather than viewing support as a stopgap measure, high-performing organizations recognize that operational stability is essential to realizing the value of transformation. This approach protects the organization from the risk that support becomes fragmented at exactly the wrong time.
Why Visibility and Governance Matter More During a Transition
Transformation initiatives succeed when leaders can see the full picture, not just the destination. But EHR transitions often reduce visibility because information becomes distributed across multiple systems, teams rely on different reporting structures, and operational insights become harder to access and act on.
Organizations need clear insight into both current- and future-state performance. They need to understand where issues are occurring, how quickly they are being resolved, how backlogs are aging, and the potential impact of those issues on clinical and financial operations.
Better visibility enables faster issue identification, stronger revenue cycle protection, more effective resource allocation, and improved operational decision-making. The cumulative impact is greater confidence and performance throughout the transition process.
Stability Is an Operating Model
The central point is that stability is not achieved by adding temporary support after a problem appears. Stability requires a structured operating model during transition.
That operating model should include domain coverage, governance cadence, escalation paths, SLA and incident management, performance reporting, optimization discipline, and accountability across both current-state and future-state workstreams.

The success of an EHR transition is defined by the cumulative effect of decisions made during implementation, not just by what happens at go-live. Informed decisions that protect current-state operations better prepare the organization for future-state value.
